Please be aware that a $150 cancellation fee applies to any appointment cancelled within 24 business hours. We also offer a 10 minute grace period for late arrivals before an appointment is considered a no-show. Thank you.

If you are an established patient, you now have access to your patient portal.  Click the link below for medication questions, refills or to request an appointment.

Our portal is available for use Monday-Friday, closing at 3pm each Friday for the weekend.

*Please note: To provide thoughtful, thorough care, we allow up to five portal messages between visits. If further discussion is needed, we’re happy to schedule an appointment.*

We no longer have an on-site lab, but we’re happy to assist you with your testing needs. Just send us a message through the patient portal to request lab orders. FOR SELF PAY LABS, please contact the office for instructions. Patients will be responsible for all lab bills not submitted correctly.
